1. Download the Tier 1 Lacrosse Camp Brochure.
2. Fill out the form in its entirety.
3. Make check payable to Tier 1 Lacrosse.
4. Mail form and check to: Tier 1 Lacrosse, P.O. Box 1403, Carlisle, PA 17013.

4. Go to our online registration page (hosted by Active.com) and complete the online registration process as instructed.
